Student Website Photo & Media Release

The Martin Warrior Golf Booster Club would like to recognize and celebrate members of the Martin High School golf program on the Martin Warrior Golf website. With parent or legal guardian permission, the Booster Club may publish a student’s headshot and limited identifying information on the website.

I give the Martin Warrior Golf Booster Club permission to publish and display photographs headshots of my child on the Martin Warrior Golf Booster Club website for purposes related to the Martin High School golf program, including player profiles, team recognition, tournament results, awards, accomplishments, and other golf-related activities.

I understand that the Martin Warrior Golf Booster Club website is publicly accessible and that photographs and information published on the internet may be viewed, copied, downloaded, or shared by others outside the control of the Booster Club.

This permission is provided without compensation to me or my child.

This authorization remains in effect while my child participates in the Martin High School golf program unless I revoke my permission in writing. A revocation will apply to future use. The Booster Club will make reasonable efforts to remove the student’s photograph and identifying information from Booster Club-controlled webpages after receiving written notice, but cannot control copies or information previously saved, shared, or republished by third parties.
